2027 LiveWell Medical Premium Discount

Catalina will offer a LiveWell Medical Premium Discount again for the 2027 plan year to continue to promote the importance of wellness and preventive care.

To qualify for the 2027 LiveWell Medical Premium Discount, employees hired on or before July 1, 2026, and who are covered under one of Catalina’s medical plans, must complete two steps by October 31, 2026. If you cover a spouse or domestic partner on Catalina’s medical plan, he or she must also complete both steps by the deadline for you to be eligible for the discount.

Employees hired after, or are covered under the medical plan following, July 1, 2026, are exempt from the LiveWell requirements and will automatically receive the discount in 2027.

Please note: If you miss the deadline of October 31, 2026, you will not be eligible for the 2027 discount.

STEP 1: PREVENTIVE SERVICE

  • Seek a preventive service between November 1, 2025 and October 31, 2026.

  • Eligible preventive services include:

    • Adult Routine Physical

    • Annual Well-woman Exam

    • Mammogram

    • Colonoscopy

    • Sigmoidoscopy

    • Flu Shot *

    • Immunization *

    • Routine Eye Exam *

    • Participate In Aetna’s Maternity Program

    • Participate In Aetna’s Health Connections Disease Management Program

* If you receive one of these services through a different insurance provider than Aetna, you must provide Catalina Benefits with proof of the visit prior to 10/31/2026 in order for service to qualify for the discount.

STEP 2: HEALTH ASSESSMENT

Complete a Health Assessment via Aetna’s new Health Your Way™ powered by ActiveHealth, (www.aetna.com), between 11/1/2025 and 10/31/2026. Detailed instructions are available in the LiveWell Medical Premium Discount FAQs.

If you cover a spouse or domestic partner on Catalina’s medical plan, he or she must also complete both steps by the deadline for you to be eligible for the discount.
